How to read H1

Total people cost ran $540.3K across H1, averaging $90.1K/month, with a low of $74.4K in April and a high of $120.4K in January.

January is the outlier month — annual/one-time items load into the January Gusto run the way they do on the P&L side, pushing payroll net to $66.1K and contractor spend to $24.9K in the same month.

Contractor (Gusto CND) spend has fallen sharply — from a $31.3K peak in February to $7.5K in June, a ~76% drop from peak. Payroll tax and benefits/REM stayed comparatively flat month to month.

June's payroll net ($53.0K) is the highest since January, consistent with the "three payroll-adjacent Gusto runs" noted on the Financial Dashboard for that month — even though June's contractor line was the lowest of H1.

Only one job title appears in this export (Medical Assistant) — it's likely a filtered or partial pull, not the full company register, since it doesn't include Mayrsohn, AlFarra, Rosario, or Timoney. The 3,937.9 reported regular hours over that ~8.5-week window works out to roughly 66 hrs/day, which is too high for a single Medical Assistant — so that hours table almost certainly reflects all roles, not just this one job, even though it sits directly under the Medical Assistant row in the source file. Worth confirming scope with Deb or Brian before using it for anything role-specific, and worth asking whether a fuller by-job or by-employee export can be pulled from Gusto directly — that would let this dashboard build a real per-role payroll section instead of the production-based proxy on the Per-Provider Detail tab.

What the mix says

Payroll (net) is the largest single line at 55% of H1 people cost, followed by employer payroll tax (22%) and contractors (20%). Reimbursements and benefits together are under 4%.

People cost as a share of total operating expense runs highest in January (73%) when annual items load into the Gusto run, and settles into the high-50s to mid-60s for most other months.

As a share of revenue, people cost is heaviest in January (>100% of that month's revenue, matching the seasonal January net loss on the Financial Dashboard) and lightest in April (38%), as revenue ramped faster than payroll that month.

What this actually measures: Maywell doesn't have a digitized Gusto employee roster yet — per the Ruotolo deal-state notes, the payroll register is still in the file cabinet. So "headcount" here is a proxy: the count of distinct billing providers (clinicians who appear on a claim) per month in the Athena export. It will miss any staff who don't bill directly — front desk, billing/RCM, business development, etc. Treat it as a clinical-panel headcount signal, not total company headcount.

What to verify with Deb/Brian

"Beall" billed Jan–Mar only (3, 5, then 1 visit, tapering to $0 paid in March) then disappears from the feed entirely. Worth confirming whether this is a covering/locum provider who stopped, or a data-entry variant of another name.

"Ullger, Cory" bills exactly 1 visit at a flat $29 every month (2 in June) — that pattern looks more like a supervising or incident-to billing code artifact than an actual separate clinician. Flag for the payroll-register review rather than counting as a 7th provider.

The 4-provider core panel (Mayrsohn, AlFarra, Rosario, Timoney) is stable across all six months — no adds or drops among the named clinicians the Analytics Dashboard already tracks.

This is production, not payroll. There's no Gusto compensation export broken out by employee yet, so this tab shows each provider's billed visits and collections from Athena — the closest real substitute for a per-role view until the actual payroll register is available. Do not read the $ figures below as anyone's pay.

What the production view says

Mayrsohn carries ~77% of H1 visits (3,461 of 4,483) and the large majority of collections, consistent with a physician-led solo-heavy panel supported by three PAs.

AlFarra's volume and collections both fell steadily through H1 — 79 visits/$11.7K in January down to 19 visits/$115 in June. That's the sharpest drop of any provider on the panel and is worth a direct check on hours, status, or a data-attribution issue before reading it as a real production decline.

Timoney's volume grew through Q1 into Q2 (15 → 40 visits Jan→Apr) before pulling back in May–June — worth cross-checking against the ramp assumptions in the forecast model's new-PA scenarios.

The June spike in "Unattributed" visits (121, vs. 17–25 in every other month) is worth a data-quality check — that's a lot of claims without a clean provider match in one month.

Updated to use ALL costs, not just payroll. "People cost" is labor only. The figures below now layer in every operating expense line (rent, supplies, insurance, EHR, marketing, etc.) and the card spend that the bank-basis P&L excludes — the All-in cost / visit row is the most accurate fully-loaded figure Maywell's current data supports.

Visits = unique patient-visit-days across all providers, Athena export. "All-in cost" = bank-basis operating expense + card spend (business + mixed) + US Bank/Amex proxy, same scope as the Financial Dashboard's Consolidated tab. The two dashed lines correct the posting-lag artifact: the green cash line divides cash posted in a month by visits rendered in it — mismatched clocks that make recent months look artificially weak while costs post in real time. Expected (history case) values every billing line at the mature actual paid-per-line for its CPT × payer class (backtested on 2025 cohorts). Expected (accrual case) additionally values unpaid NF/WC at the empirical NF fee schedule (paid-when-paid per CPT) ×1.40 for statutory 2%/mo overdue interest ×80% arbitration win rate, and unpaid PI-lien at 15% of charges. Percent-of-charges was rejected as a basis: the 2026 NF chargemaster runs 8–25× fee schedule, so charges no longer predict awards. Full method: MERGED MASTER v6, RevPerVisit Accrual sheet.

How to read this

The all-in net per visit is barely positive across H1 — $9.46 — and that blended figure hides real swings: January (–$93.13) and June (–$19.92) lost money on the average visit once card spend is counted, while March and April cleared $55–72 per visit. This is the per-visit version of the Financial Dashboard's "two nets" finding (bank-basis $159.6K net vs. all-in $42.4K net).

People cost is only 59% of the all-in cost per visit ($120.53 of $203.74 H1-blended) — the rest is rent, supplies, insurance, EHR/RCM, marketing, and card spend. A dashboard that only tracked payroll would have overstated how much cheaper April/May visits were, since non-labor costs move on a different rhythm than Gusto runs.

Visit volume was fairly steady (673–796/month) while total cost swung much more — so month-to-month cost-per-visit changes are driven by expense timing (January loading, June's buildout invoices and heavier card cycle), not by patient volume changes.

This is a company-wide blended figure, not a per-provider cost per visit — a true per-provider version would need per-employee compensation (still not available; see Notes) allocated against the Per-Provider Detail tab's visit counts.

New data source: built from APPOINTMENTS_LAST_60_DAYS.xlsx — a raw Athena appointment-status export, June 1–July 29, 2026 (2,915 scheduled appointment records). This is more current and more granular than the Jan–Jun billing feed used elsewhere on this dashboard, but covers a shorter, partly overlapping window — treat the two as complementary, not directly stackable.

What stands out

No Fault 15 is the single worst-performing appointment type — 172 of 342 scheduled visits (50.3%) never happen as booked, and it's also one of the highest-volume types. That's roughly one lost No Fault slot every day across the 60-day window.

Virtual visits hold up best — VIRTUAL VISIT 15 loses only 26.5% of bookings, well below the 38.3% overall average, and below every in-person type with meaningful volume.

Most "lost" time is rescheduling, not true attrition — 578 of 1,116 lost appointments (51.8%) are patient reschedules, meaning the patient is still coming, just not on that date. True no-shows (143, 12.8% of losses) and outright patient cancellations (236, 21.1%) are the harder losses.

The cancellation rate improved slightly from June to July (39.6% → 36.9%), though July is a partial month (through the 29th) so the comparison isn't quite apples-to-apples.

This connects to the Cost per Visit tab: every cancelled or no-show slot is a scheduled provider hour that still carries labor cost but produces no visit and no revenue. No Fault and Follow Up 15 slots — the two highest-volume, highest-cancel-rate types — are the biggest concentration of that risk.
