The objective-measurement registry (MEASURES, Layer 1): for each diagnosis, the battery — patient-reported, measured exam, labs, devices — with each piece's WC-lane role and the billing it supports. Distilled from the Degree-of-Disability Evaluation system + billing-capture map (clinical repo governs the mechanics). Rule: the code follows the note, never the reverse.
